All Independent Medical Opinions (IMOs) and record reviews are conducted strictly on an objective, evidence-based clinical basis. Provider reserves the right to decline issuing a nexus letter if the medical records and clinical evidence do not support service connection. We are not a Veterans Service Organization (VSO), law firm, or VA-accredited claims agent. We do not prepare, present, or prosecute claims for VA benefits.
Veteran Consultation and DBQ Completion

All DBQs and record reviews are conducted strictly on an objective, evidence-based clinical basis. Provider reserves the right to provide a negative medical opinion if the medical records and clinical evidence do not support service connection. Our APRN medical evaluations cover physical health conditions, secondary conditions, and chart reviews within our licensed scope of practice. In accordance with 38 C.F.R. § 4.125, initial PTSD diagnoses for VA rating purposes require evaluation by a board-certified psychiatrist or doctoral-level psychologist

Rating decisions are made solely by the Department of Veterans Affairs.

Gathering objective medical evidence shouldn’t feel like a guessing game. We utilize a meticulous, evidence-driven clinical review process to ensure your health conditions are thoroughly and accurately documented by a licensed healthcare professional.
Your journey begins in our secure Patient Portal, where you will complete your specialized medical intake forms, select your clinical service, and upload your existing medical and service records. Please note that full payment is expected prior to services rendered, and our deep clinical dive cannot begin until all documents are received.
Once your files are thoroughly analyzed, we will provide you with a finalized independent medical report documenting your diagnosed clinical conditions based on current medical science. You can independently share this objective medical evidence with your VA-accredited representative to support your ongoing healthcare and benefits documentation.
For individuals seeking to understand their clinical documentation prior to a medical examination, we offer dedicated Clinical Documentation Telehealth Consultations. These secure video sessions are treated as a separate medical review service and are booked on an hourly basis. Because complex medical histories require careful study, multiple sessions may be needed to ensure your clinical records are fully reviewed and correctly interpreted. Looking long-term, any necessary follow-up clinical sessions can be booked hourly through your portal at any time for updated medical chart reviews or ongoing clinical discussions.

Taking radical ownership of your health requires a proactive, highly collaborative partnership. To get started with our wellness and optimization programs, simply log into your secure Patient Portal to select your desired wellness path—such as advanced metabolic support, GLP-1 weight management, or peptide optimization—and choose an appointment time that works for you. From there, you will fill out your comprehensive health history and intake forms entirely within the portal. Full payment is expected prior to services rendered, and your initial consultation will be officially confirmed once all of your medical information is obtained.
During our initial telehealth consultation, we will deep-dive into your lab work, personal health history, and long-term wellness goals to craft a customized clinical protocol tailored to your unique biology. From that point forward, your entire health journey is managed seamlessly through your portal. You can easily book your follow-up options, track your metabolic progress, and communicate directly with me through secure messaging whenever you need support.

Financial and Fee Model Transparency
All fees for Independent Medical Opinions, chart reviews, and telehealth consultations are flat hourly or fixed per-evaluation rates for medical time only. Grace and Grit Wellness LLC does not charge, accept, or participate in contingency fees, percentage-based fees, or success-based compensation tied to VA disability claim outcomes or rating percentages.

Telehealth Jurisdictional Scope and HIPAA Compliance
Telehealth clinical consultations and physical examinations are restricted to individuals physically located within the State of South Carolina (or states where the provider holds an active APRN license) at the time of the appointment. Out-of-state chart reviews are conducted solely as medical-legal document reviews under federal guidelines.
